The hypothalamic-pituitary-adrenal axis is activated during sepsis to increase cortisol concentrations; however, the secretion of cortisol can be insufficient for the severity of disease in a number of sick foals. Our group and others have shown that relative adrenal insufficiency (RAI), characterized by a poor cortisol response to adrenocorticotropin (ACTH), is associated with mortality and severity of disease in foals. Most studies investigating RAI in equine patients have been focused on cortisol, while other adrenocortical steroids have been overlooked.
