Document: The questionnaire was first distributed online on March 14th 2020 at 14:43 (Israeli time zone, GMT+2) and was spread via social media and press. To this date, March 21st, 18pm, there have been 54,059 responses, including 50,572 (93.55%) adults and 3,488 (6.45%) children. The characteristics of the responders are described in Table 1 , and by response date in Table S1 in the Appendix. Altogether, 2,216 (4.1%) responders reported that they are currently in isolation. For those reported as being in isolation, 1,116 (50.36%) responders were due to a recent international travel and 1,100 (49.63%) were due to a contact with an individual who was infected with COVID-19 or an individual who recently returned from any destination abroad. A new version of the questionnaire was established on March 21 driven by new policies that have been implemented by the ministry of health ( Figure 2 ) and a comprehensive meta-analysis of 14 published papers on patients with COVID-19 symptoms (7 of which were on patients over 18 years old and 7 on patients under). By that time, new data was published which we relied on instead 6 . This version includes several more questions ( Figure 1 ) and was not distributed yet.
