Almost all patients have multiple lesions in the deep and subcortical white matter, while the periventricular white matter is generally spared. The thalami and basal ganglia are frequently affected (Fig. 2) , and lesions in these locations are often symmetrical 3, 22) . Brainstem and spinal cord abnormalities on MRI are common in ADEM 12) . The number of lesions varies, and their diameters range from <5 mm to 5 cm 1,3) . Values are presented as %.
