Of the 268 patients who had received prior antimicrobials, 77.6% (n = 208) had a bacterial pathogen detected by PCR, but only 32.1% (n = 86) were culture positive (P < .0001). The mean combined load of all detected bacteria per patient was significantly higher in patients who had not received antimicrobials prior to sampling (5.40 × 10 9 CFU/mL) compared with those who had received antimicrobials (2.67 × 10 8 CFU/mL; P = .0001).
