According to the Law, a case of AGI has to be notified by the treating physician using a standard notification form. The data collected on the notification form include: a name, surname, date of birth, permanent address, notification date, and disease code according to the International Classification of Diseases, 10th revision (ICD-10). The data is collected at the regional level, then sent to the National Institute of Public Health and entered into SURVIVAL (the national electronic database of communicable diseases in Slovenia).
