All patients will receive standardised surgical processes and perioperative care under existing protocols for postoperative ICU sedation, analgesia and weaning from mechanical ventilation. Also, all patients will be given a standardised anaesthesia, described in detail elsewhere, 31 with volume-controlled ventilation set at a tidal volume of 8 mL/kg of ideal body weight with a positive, end-expiratory pressure of 5 cm H 2 O.
