Data were entered into a password-protected Excel spreadsheet, and analyzed using Stata 11.0 (STATA Corp, College Station, TX). Comparison of clinical characteristics was performed using Fisher's exact tests for categorical variables and t tests with unequal variance for continuous variables. This study was approved by the Seattle Children's Hospital IRB.
