Document: Over 12 years the development and redevelopment of internationally recognised training standards for respiratory medicine have met with many ERS international Adult Respiratory Medicine syllabus for postgraduate training challenges and, at times, limitations for further development. On the one hand, this ERS task force has respected and supported the wide variations of standards and regulations of each individual country in their best efforts to provide trained and certified respiratory medicine specialists. On the other hand, the mobility of medical professionals and the increased mobility of patients have placed increased pressure on medical facilities to provide and meet minimum international standards of quality patient care. Attaining and managing this balance has been no easy feat. The work of this international task force and the Delphi process has ensured that the individual voice of each represented country has been heard, taking into account the various needs and respective perspectives of training currently in operation (table 1) . These inherent challenges have called for a number of considerations.
