Document: The mechanisms underlying virus-induced exacerbations in COPD and the factors that drive inflammation and clinical severity during these episodes are poorly understood. Here, we provide insight into the role that the major airway mucin glycoprotein MUC5AC plays in the biology of these episodes. Using a combination of human and mouse models of rhinovirus infection in COPD, we demonstrate a novel central role of virus-induced MUC5AC in driving COPD exacerbation severity through amplification of airway inflammation, findings that open up new opportunities for targeting MUC5AC therapeutically. We indicate a mechanism for MUC5AC augmentation of RV-induced inflammation via release of extracellular ATP.
