Document: Clinical echocardiogram reports followed American Society for Echocardiography (ASE) guidelines which allowed grading of VHD as follows: none, mild, moderate, or severe. 23, 24 The echocardiography laboratories at our study sites included additional categories of trivial, trivial-tomild, mild-to-moderate, moderate-to-severe, and critical. The final clinical reports of the subject echocardiograms were de-identified and associated with study subjects at the study sites, with particular attention to the presence and degree of valvular stenosis and regurgitation of each of the four valves. For our disease screening evaluation, we define "clinically important" or "significant" VHD cases as those graded moderate-to-severe or worse, for this would encompass all levels of disease which could require timely intervention beyond serial monitoring. We define controls as subjects free of valvular, structural, or congenital heart disease, with no valvular regurgitation or stenosis beyond trivial or physiologic severity.
