Studies in Europe and the United States have shown that 40% of children live with a smoker [34] and they have approximately twice the risk of contracting a serious respiratory tract infection in early life [91] . Cigarette smoking leads to a longer duration of cough, greater frequency of abnormal auscultatory findings during acute respiratory tract illness [92, 93] and higher risk for severe exacerbations [94] . Urinary leukotriene E4 levels identify children exposed to ETS at high risk for asthma exacerbation [94] .
